What to bring
Personal Identification & Documents
- Photo ID (driver’s license, state ID)
- Insurance cards (Medicare, Medicaid, private insurance)
- Copy of Advance directives / Power of Attorney documents
- List of current medications, dosages, and medical history
- Emergency contact information
Personal Hygiene Items (if you wish, generic products will be provided for you, but for your comfort bring what you wish too.)
- Toothbrush, toothpaste, floss
- Hairbrush/comb
- Shampoo/conditioner
- Body wash or soap
- Deodorant
- Razor and shaving cream
- Lotion or moisturizer
- Facial tissues
- Denture care supplies (if applicable)
Clothing
- 7 day supply of Comfortable, loose-fitting clothes (T-shirts, sweatpants, shorts)
- Undergarments (enough for several days)
- Socks and non-slip slippers
- Pajamas or nightgowns
- Bathrobe
- Sweater or light jacket
- Shoes with good support (preferably Velcro or slip-on)
- Outfit for discharge day
Medications
- Medications list.
- Only bring medications if specifically requested by facility.
Comfort & Personal Items
- Earplugs or sleep mask (if sensitive to noise/light)
- Photos of loved ones
- Reading materials (books, magazines)
- Glasses, hearing aids, dentures + storage cases
- Cell phone and charger (and headphones if needed)
- Notebook and pen
Rehab Aids
- Walker, cane, or wheelchair (if already in use)
- Braces, orthotics, or prosthetics (if applicable)
- Comfortable athletic shoes for therapy
Electronics
- Cell phone, tablet, or laptop
- Chargers and power strip
- Headphones or earbuds
Miscellaneous
- Laundry bag (for soiled clothes only if you or your family will be doing laundry.)
- Labels with name (for clothing and valuables)
- All items will be labeled while at the facility, the facility can help label items if needed.
